The ADULT
DAY WELLNESS PROGRAM, a
licensed adult day program located at ALOHA NURSING
& REHAB CENTRE, 45-545 Kamehameha
Highway, Kaneohe, Hawaii, is a special program for older adults. This
program provides exercise and recreational therapy, as well as social, educational
and cultural activities during the day in a supervised, therapeutic
environment. Hours of operation are Monday through Friday 6:45 a.m.
to 6:00 p.m. The program is closed for most
state and federal holidays.
Our MISSION is to enhance the quality of life by providing a
structured, therapeutic environment promoting overall wellness.
*CRITERIA FOR ADMISSION:

- Exercise and Recreational Therapy
- Meals - Breakfast, Lunch and a Snack
- Cultural Activities and Excursions 3-4x/week
- Social Services Assistance
FEES:
Daily rates based on client's level of functioning
- Level I (Independent) - $50/day
- Level II (Supervision) - $55/day
- Level III (Minimum Assistance) - $65/day
- Level IV (Moderate to Maximum Assistance) - $75/day
 OTHER
COSTS:
- Intake/Processing Assessment Fee - $20.00 (non-refundable)
- Deposit - $100.00 (refundable at time of discharge, pending damages/accrued
expenses)
- Off-Site Activity Expenses (excursions)
